Once Upon a Small Town episode 6 starts with a glimpse into Ji-yul’s trauma and how he got into a horrible accident years ago wherein he lost his parents. Back in the present, Ji-yul rescues Ja-young from the wreckage in the exact same way in which her father had once rescued him years ago.
In the hospital, Ji-yul looks over Ja-young as she lies unconscious in the hospital. Just as he’s about to hold her hand, Sang-hyeon gets there and thanks Ji-yul for helping his friend. With Sang-hyeon there, Ji-yul leaves the hospital looking rather morose, while Ja-young wakes up from her sleep and assures Sang-hyeon that she is fine.
When she asks him how he got there, he tells her how Ji-yul reported the accident and helped her get to the hospital and then teases her about owing the doctor for his kindness. When Sang-hyeon helps her out of the hospital and to his car, Ji-yul sees how close they are and leaves without saying anything.

At home, Ja-young tries to find the right words to thank Ji-yul, but isn’t able to translate her feelings. Meanwhile, at the peach farm, Sang-hyeon remembers back to seeing Ji-yul waiting in the hospital parking lot and has some things to think about. Similarly, Ji-yul, too, has thought of his own.
The next day, Ja-young gets booted out of work by her chief so that she can rest, and she takes the free time to meet Ji-yul and thank him for the other day. However, Ji-yul isn’t at the vet clinic, and thus she leaves some popsicles for them on her way to another neighbour’s house. Ji-yul, meanwhile, has gone to his parent’s columbarium after the activities of the last day.
While on their way back, Ja-young and Ji-yul come across each other, and the meeting is rather awkward for them both. After the initial confusion, Ja-young starts to thank him for his help, but that gets cut short when he gets called somewhere urgently. As Ja-young looks at Ji-yul’s fading car, Sang-hyeon picks her up, and they go back to talking about the activities with Majeong. He later cheerfully asks her to think some more when she mentions what he had said about his feelings for her.
Meanwhile, Ji-yul goes to an old man’s home to check on his bull. Unfortunately, the bull isn’t doing well, and the man wants Ji-yul to euthanise the bull, who hasn’t eaten anything for a month now and take him out of his misery. On the other hand, a brawl breaks out between Majeong and Huidong that night during a meeting between its residents.

While those from Majeong call Ja-young uncultured because she doesn’t have parents, Ji-yul must make the difficult decision of figuring out the perfect medicine to ease the bull’s pain and put an end to its misery. However, Sang-hyeon does at least try to ease Ja-young’s burden, if momentarily.
As she goes on her walk to clear her head, Ji-yul helps the bull pass over, and the old man’s loving words to his long-time friend bring tears to the vet’s eyes. Later on, the man talks to Ji-yul about his parents, and as it turns out, he had taken care of Ji-yul during the tragedy all those years ago. They somewhat bond over their individual tragedies, and the old man talks about how those that we have lost will definitely come back to us somehow or the other.
Just before Ji-yul leaves, however, the man mentions that he used to play with Ja-young when they were young, opening up a new load of memories and clearing things up for him further. He finds her sitting by the river and remembers the time when they were young, when Ja-young had befriended him and helped him get over the heartbreak of losing his parents.
As the memories flood back, Ji-yul and Ja-young share a look as they are standing in front of each other right at the spot they used to hang out at when they were little.
Well, now that we are not forcing anyone to do anything they don’t want, episode 6 has turned out to be a rather heartwarming episode. I think most of the audience was waiting for Ji-yul to remember Ja-young finally, and I must say, I loved the way they brought the story back to him. That old guy with the bull almost brought me to tears, and it makes sense that he knows Ji-yul and his relationship with Ja-young from all those years ago.
